Hidden behind other world quests, under The Subterranean Trials of Drake and Serpent quest.
One such quest is The Heart of Ouroboros. Upon completing this quest, you will receive one of the three fragments that you need to complete the second trial.
Though, during the quest, you get to a section where you have to solve the riddle at the altar in the Serpent’s Heart to get the fragment.

Examine the Mechanism and Open the Gate to the Secret Room
In this section of The Heart of Ouroboros world quest, you will find a hidden message on the ground with the five key sigils. To solve the riddle, you have to pass through the barriers with the key sigils on them.
When you pass through the barrier with the key sigil icon, it will start glowing—which means it’s activated.
Now, pass through all five barriers, and you will solve the riddle at Serpent’s Heart. Once that is done, an exquisite chest spawns. Open it to get one of the three fragments.
