Where to Get Unalloyed Gold Needle
You can get the Unalloyed Gold Needle by defeating the side-boss Commander O’Neal found in the Swamps of Caelid region in the Lands Between.

You can find Commander O’Neal in the Aeonia Swamps, found southwest of the Sellia Town of Sorcery.
If you are having trouble finding Commander O’Neal, refer to the map screenshot. Once you are at the boss arena, defeat Commander O’Neal to get the Unalloyed Gold Needle. Try to avoid his summons, as they can easily overwhelm you with attacks.

Once you have the Unalloyed Gold Needle, you can take it to Sage Gowry and get it repaired. After that, head to the Church where Milicent is located. Give the needle to Milicent to cure her rot.
