To reach the Primeval Sorceror Azur Site of Grace, teleport to the Schoolhouse Classroom Site of Grace and take the path that leads to the spinning elevator.

Use the elevator to reach the bottom part of the location. There you will find one of the Adjudicators, get close to him, and let him kill you with his special attack where his chest to stomach opens up. And hands pull you in. 

Once you are dead, you will find yourself at the Volcano Manor, explore the area till you reach the Site of Grace. From there, make your way to the boss room, defeat the boss and get out of the cave.

You will find yourself in a secret part of Mt. Glemir. Light up the Site of Grace at the bottom and then move forward. Beware of the erupting ground. Keep moving till you find a lava floor and a Magma Wywrm boss. 

Take left, get past the boss and keep moving. You will eventually reach the Hermit Village. In the Hermit Village, right past the boss, you will find Primeval Sorceror Azur Site of Grace.
