You can change your character’s appearance by using the Clouded Mirror Stand found in the chamber of the NPC Fia, Deathbed Companion, in the Roundtable Hold.

Unlocking the Roundtable Hold—the hub area for the Tarnished—is an easy task. First, go and interact with the Gatefront Site of Grace to summon Milena. Once that is done, go to Stormhill Shack and interact with the NPC Roderika [Optional].
Now, interact with rest at the Stormhill Shack Site of Grace. Milena will spawn automatically. Exhaust her dialogues. And at the end, you will be teleported to the Roundtable Hold.

Head to Fia’s Room, and you will find the Clouded Mirror Stand on the right side.
Interact with the mirror and select the option to “Apply Cosmetics” to change your character’s appearance.
