The process is relatively straightforward, but there are some things to watch out for to maximize it and create the best possible items.

How to Synthesize Items in Atelier Sophie 2: The Alchemist of the Mysterious Dream
Synthesizing items in Atelier Sophie 2: The Alchemist of the Mysterious Dream can only be done inside the Atelier. To get things started, interact with the cauldron and pick either Synthesis or Plachta’s Synthesis. The differences between the two options are limited, as only a few select items can be synthesized exclusively by Plachta. This option will also not be available from the start of the game, so do not worry if you haven’t unlocked it yet, you eventually will automatically as you make some progress.
After interacting with the cauldron and picking your Synthesis option, you will be asked to select an Alchemy Recipe and the required ingredients. The higher the quality of the picked ingredients, the better the quality and effects of the synthesized item will be.
Once you have picked the ingredients, you will have to place them on the Synthesis Panel. Each ingredient fills the board differently and has a different elemental property. If you manage to place your elements on the board properly, and even fill lines both horizontally and vertically, you will achieve a Super Success, which greatly increases the quality of the synthesized item. If you do not care about this, and want to get things done quickly, just press the ZR button to auto-add ingredients.
After ingredients have been placed on the Synthesis Board, you will be able to pick traits. Up to three trains from the used ingredients can be added to the item, and depending on which you pick, some may be fused together for increased efficacy. Some traits can only be received by fusing them, so pick the right ingredients, and you will be able to get powerful items that cannot be obtained in any other way!
